For your safety, we ask that you do not drive to the clinic, due to severe road conditions. All Tuesday, Wednesday, and Thursday appointments (Jan 21 - 23) will be changed to Tele-health. Your appointment time stays the same. Please dress comfortably and fully charge your cell phone for your appointment.

Instructions:

1. Download Zoom app

2. You will receive a text to connect to your secure Zoom call. Please be ready to click the link, around your appt day & time on your phone. (Due to the condition. There may be delays. Please be patient.)

3. The delay may be up to 1 to 2 hours depending on the connection and operation.

4. If you did not receive a text or a phone call, please use the patient portal to text before 4:30pm.

The patient portal is the fastest way to reach us for urgent matters. Should you have any issues after 4:30 pm, please contact us through Facebook.

Please! Do not overload the communication system, as that may delay the quality of care we provide to you.

Preparing for your Zoom Call

Be ready to receive a phone call from our front desk.

1. Confirm your appt time on Tuesday or Wed

2. Prepare your pre-payment of copay and/or deductible.

3. Update your new insurance information.

4. Make sure to give your best phone number, so you can receive the text to connect to our Zoom call with you.

We have a new patient portal that has multiple features to answer most of our patient questions. We have staff available to assist patients with registering for the portal, and it only takes 5-10 minutes on average to register.

Once registered, patients have access on the portal to:

  • View appointment details like location, date, and time

  • Update demographic information to save time at check-in

  • Upload insurance cards and IDs

  • View and download medical records

  • Contact your provider directly

  • Ask questions about billing, prescription, and preauthorization

  • View your balance and pay your bill online

    • Request or change Appointment

  • Set notification and appointment reminder preferences

COVID-19 TREATMENT SERVICES

If allowed by the insurance company, all patients that show COVID-19 symptoms should avoid face-to-face contact visits and instead

follow-up through our Telemedicine Services.

Cervical, thoracic, lumbar, and caudal epidural injections:

An epidural steroid injection (ESI) is a combination of a corticosteroid medication with a local anesthetic. Corticosteroids are strong anti-inflammatory medications used for pain relief. The local anesthetic causes immediate pain relief, while the corticosteroid allows long-term relief. The ESI is injected into the space around the spinal cord, which is called the epidural space. 

Trigger point injections:

Trigger point injection (TPI) is a method used to treat painful areas of muscle tissue that contain "trigger points," or knots of tightened muscle that cannot relax. These knots can irritate surrounding nerves and cause pain in different parts of the body.

Facet injections:

A local anesthetic or steroid medication is injected into a facet joint, which is one of the points where one vertebra (spine bone) connects to another.
